I had significant problems with OS and OALD when DS was born 2.5 years ago. Long story, but I experienced almost weekly plugged ducts and he had severe colic problems for the first 18 weeks until things finally leveled off . DD is now 4weeks old, and I started block feedings as soon as my milk came in at day 3. Nursing is better this time around, but I am still having problems. I am currently nursing in 4 hour blocks, but DD still chokes and gags when nursing and frequently pulls off the breast. She will also spit up very large amounts about once to twice a day. I know this is due to OALD. I will hand express some milk once I feel the let down (or before nursing if I feel engorged). I can easily hand express a couple ounces before the milk stops spraying (usually I just let it spray into a towel). I want to help reduce DD's discomfort, but won't expressing this much just continue to stimulate more milk production? I am also nursing her while either reclined or laying down with her on top to try to reduce her choking. She is otherwise doing well, having plenty of wet/poopy diapers and gained 20oz over the past 13 days . I will also be returning to work in a couple months so I have pumped twice so we can start introducing DD to a bottle. I was able to pump 4oz from one breast in less then 5min .
