My LC suggested using a pea-sized amount of 1% hydrocortisone cream and Bacitracin ointment (apply with a clean finger, obviously). The hydrocortisone combats inflammation and pain and the Bacitracin fights bacteria. My LC said it did not have to be washed off after nursing.
You could also try immersing the wound in a shot glass full of warm water for about 30-60 seconds before latching the baby on, or hydrating with a really wet washcloth. That will really soften the scab and reduce pain, because pain is greatest when a tough, dry scab tears.
