    Default Engorgement with no milk....

    Hello all, I'm very new to forums but have used them in the past to obtaun info, but now I'm in need of serious help. My daughter is 3 days old and I have no milk or colostrum. I've been pumping and nursing and still nothing comes out. 3 years ago I had breast augmentation surgery and my surgeon assured me it wouldn't interfere. My implants are under the muscle and thru the botton half of my nipple. I breastfed 2 kids successfully prior to my augmentation. I feel my milk has come in and I am seriously engorged, but nothing comes out of my nipples. I squeeze, pump and even had my husband suck on my breasts and nothing comes out of the nipples. Has anyone else had this problem and what can I do? I am in so much pain and I keep feeling the milk let down. I have cried for 3 days now because of the fact that I may not be able to breastfeed my daughter. She is on formula now. Please, please someone help me.

    Can you please describe where your implants are located again? The implants are under your chest muscle? What technique did your surgeon use (through the armpit, under the breast, through the belly button)? And when you say "thru the bottom half of my nipple" what do you mean? Was your areola (the dark area around your nipple) cut?

    Is there an IBCLC on staff at the hospital? What did she say?

    If you are not currently seeing an IBCLC (International Board Certified Lactation Consultant), I urge you to see one now.     Here is what I found out:

    The scar along the bottom of your areola could possibly cause problems with let-down, BUT should not prevent the milk from coming out by negative pressure (sucking). VERY GOOD NEWS!

    Edema (swelling) could be preventing the milk from moving out. The vacuum action of the pump pulls more fluid into the region. Hand expression helps you to remove milk without that concern. Here's how to do it:

    In the meanwhile, it would be beneficial to spend lots of time in skin to skin contact with your baby (baby stripped down to diaper, you topless, baby upright on your chest, blanket around you both for warmth). This will help both of you feel more comfortable and relaxed. Your baby may even decide to nurse once she gets there.

    Are you currently nursing at all? Did baby nurse well from the start? Did he urinate and pass meconium after nursing? At what point did you begin supplementation?
