    Thrush can be really stubborn. Sometimes nystatin isn't enough. You might want to look into Diflucan and/or gentian violet. Another possibility is a secondary bacterial infection in the crack- you might want to try a little Bacitracin ointment on the crack and see if it makes a difference.

    Wouldn't the ointment cover bacterial infection as well? Or should I add bacitracin to the regimen as well?

    Oops, I missed the fact that you were using APNO. That should cover bacteria as well as yeast.
